TRS Forms
Forms are provided in .pdf format. If you are unable to view them, you can download the free Acrobat Reader software from the Adobe website: Get Acrobat Reader DC.
(Don't have a printer? Call TRS to have a form mailed to you.)
Many forms are designed with fields you can type in. Follow these instructions:
1.) Find the form you want within the categories below
2.) Click View/Print to open the PDF document in a new browser tab
3.) Save the PDF to your computer's hard drive before you begin typing
4.) Type your information, and be sure to click Save again
5.) Print the form, sign as instructed, and mail to the address shown on Page 1.
